ELDERS AND SENIOR CASE MANAGER

Aseniwuche Winewak Nation of Canada, A Fellowship of Aboriginal People, or AWN, is a provincially incorporated not-for-profit society based in Grande Cache, Alberta. AWN members share a unique history and ancestry as descendants of a diversity of Indigenous groups of Canada including Cree, Iroquois, Sekani, Dene-saa, Ojibwe, Assiniboine, and Shuswap. The Aseniwuche Winewak Nation are a Cree speaking people whose ancestors thrived in the Rocky Mountains and members continue to live a distinctive cultural way of life.

Position Overview:

Under the direction of the Preventative Social Programs Manager, this position will help develop and implement a Collaborative Community Response (CCR) to address Elder abuse that addresses the unique needs of both our Indigenous and Non-Indigenous people; provide support, information, referrals, and advocacy to Elders/Seniors and their families to address Elder abuse and neglect, and work with the Tawow Centre team to support the cultural, mental, physical, emotional, and spiritual well-being of our community.

Duties:

  • Plan, coordinate, and/or facilitate a Collaborative Community Response (CCR) and programs that support Elders/Seniors safety and well-being.
  • Utilize the Action Against Elder Abuse-Community Response Project Case Management Tools, Best and Promising Best Practices and Support Mechanisms.
  • Screening inquiries as to need for intervention; contact police in emergent circumstances or potential criminal matters.
  • Refer non elder abuse inquiries to appropriate community resources.
  • Conduct intake and assessment interviews with potential clients.
  • Create and implement case plans for clients requiring on-going support.
  • Advocate on behalf of clients accessing services and supports.
  • Support police and victim services when required.
  • Report to stakeholders and funding bodies, including regular meetings with steering committee.
  • Create data collection and evaluation process; administer as applicable.
  • Develop and maintain professional relationships with the support community; assist in creation of Memoranda of Understanding and formalized partnerships.
  • Incorporate traditional cultural practices into Elders/Seniors wellness and safety programs.
  • Apply AWN’s Diversity Lens and Programs Plan to ensure the programs meet identified strategies and demonstrate a commitment to Reconciliation.
  • Utilize the Elder Knowledge Keepers/Helpers as a program resources.
  • Provide outreach, social programs, and traditional teachings to relieve Elders/Seniors loneliness, isolation, and vulnerability thereby supporting their cultural, physical, emotional, mental, and spiritual well-being.
  • Assist Elders/Seniors and/or their families to find solutions to their concerns.
  • Provide information and supportive referrals to local and outlying service providers and Government resources including assistance with forms.
  • Assisting at-Risk Elders/Seniors and/or their families with safety planning.
  • Ensure the program is accessible by meeting Elders/Seniors in their homes.
  • Coordinate, promote and deliver Elder Abuse Awareness and support, and It’s Not Right! Friends, Family, Neighbours of Older Adults Bystander training.
  • Research current issues that are impacting Elders/Seniors and facilitate community and organizational discussion on how to fill identified gaps.
  • Provide referral supports to at-risk Elders/Seniors and their families seeking seniors housing including lodges, long-term care, and supportive living facilities.
  • Conduct surveys and interviews with CCR members, elders, and their families to determine if the CCR is appropriate and meeting the targeted need.

Qualifications:

  • Minimum post-secondary diploma in Social Work, Human Services, Gerontology, or equivalent education and experience.
  • Minimum 2 years’ experience developing, coordinating, and/or facilitating Elders/Seniors programs and services.
  • Proven expertise in building relationships with Elders/Senior clients, their families and community organizations.
  • Experience working with seniors with dementia; Alzheimer’s Disease; mobility issues; sight and/or hearing loss; addictions, and/or mental illness.
  • Fluent in English and the local Cree dialect.
  • Ability to remain calm and non-judgmental in all situations and in all dealings with clients, families, service providers, and community members.
  • Keen understanding and knowledge of local Indigenous culture, community values and traditions.
  • Experience developing, coordinating, and facilitating Elders/Seniors programs that support their culture, traditions, and well-being.
  • Ability to network and promote Elders/Seniors programs and services to the community and organizations on a consistent basis.
    • A thorough understanding of community/rural issues that affect Elders/Seniors and knowledge of appropriate supports.
    • Strong working knowledge of Microsoft Office Applications (Word, Excel, Access, Outlook, and PowerPoint).
    • Excellent organizational and time-management skills.
    • An ability to work independently and in a team environment.

Certifications, Licenses, and/or Designations:

  • Current First Aid, CPR & AED Certification.
  • Applied Suicide Intervention Skills and training on working with clients with dementia and/or Alzheimer’s Disease would be an asset.
  • Class 5 license and own reliable vehicle; $2,000,000 vehicle liability business insurance.
  • A clear Criminal Record/Vulnerable Sector Check required before commencing employment. 

Communication and Interpersonal Skills:

  • Possess strong oral and written communication skills.
  • Good public speaking and facilitation abilities.
  • Exceptional interpersonal skills that forge respectful, positive interactions and relationships with Elders/Seniors, their family members, and service providers.
  • Ability to cope and have standard of practice solutions for situations faced by vulnerable clients.
  • Ability to cope with a fluid and dynamic schedule.

 

Work Demands:

  • Typical office environment.
  • Sitting for short periods in an office environment and while driving.
  • Standing and moving around for extended periods while delivering programs and/or interacting with participants.
  • Moderate lifting not to exceed 30 lbs.
  • Outdoors in land-based settings.
  • Rural and Town Home visits and meetings with partner agencies.
